FindMyDirectDoctor, LLC, which owns and operates findmydirectdoctor.com ("FindMyDirectDoctor," "we," and "us"), uses your personal and personally identifiable information to provide our services to you, to customize our services to your preferences and requests, and to communicate with you. If we provide you with interest-based advertising or utilize third-party service providers in a manner that would constitute a "sale" or "sharing" of personal information under the California Consumer Privacy Act of 2018, as amended by the California Privacy Rights Act (collectively, "CCPA/CPRA"), you have the right to opt out using the methods below. As of the Last Updated date, FindMyDirectDoctor does not sell personal information for monetary consideration and does not share personal information for cross-context behavioral advertising as those terms are defined under the CCPA/CPRA. FindMyDirectDoctor does not practice medicine; clinical services are provided by the Healthcare Provider.
The CCPA/CPRA does not apply to Protected Health Information ("PHI") that FindMyDirectDoctor handles in its role as a HIPAA Business Associate of OpenLoop Healthcare Partners, PC and its affiliated professional corporations (the "Healthcare Provider"), which is the Covered Entity. PHI is governed by the Health Insurance Portability and Accountability Act ("HIPAA"), the California Confidentiality of Medical Information Act ("CMIA"), and other applicable health-information laws, not the CCPA/CPRA. To verify your request, we may ask you for information to confirm that you are a user of our services, such as your phone number, email address, city, state, or geographic location.

To do so, you must provide us with proof that the individual or business has been appointed as your agent, such as by providing a signed power of attorney form. We may also request information from you to confirm that you are a user of our services.
California residents have the right not to receive discriminatory treatment for exercising any of the rights conferred under California law.

To the extent of any conflict between this page and the Terms of Service on those matters, the Terms of Service govern. Any dispute arising from or relating to this page, including claims under HIPAA, CCPA/CPRA, CMIA, the Washington My Health My Data Act, Nevada SB 370, the Texas Data Privacy and Security Act, or any other state or federal privacy law, is subject to Section 20 of the Terms of Service.
