Telehealth Informed Consent

The Telehealth Services delivered through findmydirectdoctor.com are provided by OpenLoop Healthcare Partners, PC and its affiliated professional corporations (collectively, the "Healthcare Provider"), as further described in Section 8 of the FindMyDirectDoctor
Terms of Service. FindMyDirectDoctor is a technology platform and is not a medical provider; FindMyDirectDoctor does not deliver clinical care, does not prescribe medication, and does not draft or administer the informed consent document for telehealth treatment.
Informed consent for telehealth treatment is collected by the Healthcare Provider at the time of clinical intake. Before you receive any telehealth visit or prescription, the Healthcare Provider will present a full informed-consent document that you must review and affirmatively accept. The Healthcare Provider's consent covers, among other matters, the nature and risks of telehealth, treatment-specific consent for compounded medications and other therapies, the use of artificial intelligence and ambient-listening technologies in clinical care, recording of telehealth visits, the use and disclosure of your protected health information, billing and assignment of benefits, authorization for SMS and email communications, and state-specific disclosures and complaint procedures for residents of certain states.

That document, as it may be updated from time to time by the Healthcare Provider, controls the terms of your informed consent for telehealth treatment. This Page is informational only and does not modify, replace, or supersede the Healthcare Provider's consent document. If the Healthcare Provider's consent document and this Page conflict, the Healthcare Provider's consent document governs.
This Page is incorporated into and forms part of the FindMyDirectDoctor
Terms of Service. Your use of the Services, including all matters relating to the delivery of telehealth care by the Healthcare Provider, is governed by the Terms of Service, including without limitation Sections 8 (Telehealth Services), 18 (Disclaimer of Warranties and Limitation of Liability), and 20 (Resolution of Disputes and Governing Law).
